Funds Provide Services for Formerly Homeless Families, Veterans
and Individuals with Special Needs at the Jersey Shore
Red Bank-based nonprofit HABcore, Inc., which provides housing and supportive services on the Jersey Shore, has received a $15,000 grant from The Provident Bank Foundation to support the Independence Pathways Program (IPP).
“As the cost of housing continues to climb out of reach of many people, particularly those who face health challenges and the chronically homeless, this grant from The Provident Bank Foundation will help us meet those needs,” said Steve Heisman, President and CEO. “Our Independence Pathways Program, which focuses on households with disabilities, delivers individualized care plans that address housing needs, but also the supports that help with stability, address health struggles and provide a path to greater independence.”
Focusing on formerly homeless households with disabilities, many of which are families with children, HABcore’s IPP is the agency’s fastest-growing initiative. The program, which combines affordable housing with coordinated supportive services, assists households experiencing homelessness and struggling with chronic physical and mental health issues. HABcore’s Supportive Housing Coordinators provide hands-on case management with access and referrals to a variety of resources including social services, employment, education, transportation and healthcare, along with budgeting, life skills, and parent and communication training, as needed. Collectively, these supportive services deliver measurable outcomes and positive, long-term stable housing for more than 98% of participants.
