HABcore Receives $25,000 Grant from Horizon Blue Cross Blue Shield of New Jersey to Support the Nonprofit’s Independence Pathways Program
Funds Provide Services for Formerly Homeless Families, Veterans
and Individuals with Special Needs at the Jersey Shore
Red Bank-based nonprofit HABcore, which provides housing and supportive services at the Jersey Shore, has received a grant in the amount of $25,000 from Horizon Blue Cross Blue Shield of New Jersey. The donation, made through The Horizon Foundation for New Jersey, will be used to fund the organization’s Independence Pathways Program (IPP).
“This major grant from Horizon will help us to provide critical supportive services for our Independence Pathways Program,” said Steve Heisman, President and CEO of HABcore. “With these funds our IPP participants receive individualized care that meets their needs and leads to their greatest level of independence.”
With an emphasis on formerly homeless households with disabilities, many of which are families with children, HABcore’s IPP has emerged as the agency’s fastest-growing initiative. The program combines affordable housing with coordinated supportive services to assist households experiencing homelessness and struggling with chronic physical and mental health issues. HABcore’s Supportive Housing Coordinators provide hands-on case management with resources, guidance and connections to community resources, social services, employment, education, transportation, healthcare and financial literacy, as well as life skills training. Collectively, these support services deliver measurable outcomes and positive, long-term stable housing for more than 98% of participants.
